AUTONOMIC NERVOUS SYSTEM PHARMACOLOGY-CHOLINOMIMETIC DRUGS

Drug:  Bethanechol 

Mechanism of action and drug effects:  Muscarinic agonist with negligible effect at nicotinic receptors.  Activates M1 through M3 receptors in all peripheral tissues.  It causes increased secretion, smooth muscle contraction (except vascular smooth muscle relaxes), and changes in heart rate. 

Clinical applications:  Postoperative and neurogenic ileus and urinary retention. 

Pharmacokinetics, Toxicities, Interactions:  Oral and parenteral, duration approximately 30 min, does not enter central nervous system (CNS).  Toxic effects: Excessive parasympathomimetic effects, especially bronchospasm in asthmatics.  Interactions: Additive effects with other parasympathomimetics.
